Getting Started: To get started in the CSBR group, call the Intake Team at 612-377-9190 x 1; be sure to have your insurance information with you at that time. The Intake Team will gather a variety of important information for Dr. Scott Jacoby, the group coordinator, as well as schedule an initial, individual intake appointment with Dr. Jacoby, in order to determine placement in the group.
