How do I get started as a new client?
Those interested in setting up an appointment at Kenwood should call 612-377-9190, ext. 1. You will go through a Confidential Phone Intake that includes the following information:
- Date of Birth
- Contact Information (Phone number, address, e-mail if applicable)
- Any Applicable Insurance Information (Name, ID and group number)
- A brief description of any presenting problems or concerns
An intake coordinator will use this information and any additional requests to match clients with a counselor. They will do their best to set up your first appointment with a clinician who can best fit your needs.
How long will it take?
We do our best to schedule clients with their first appointment within the first phone call. In addition, counselors will be available within the next week unless otherwise stated up front. Some counselors may be available as soon as the next day.
What should I bring to my first session?
Having your insurance card, insurance information and completed new client paperwork (which can be found below) ready when you arrive at your first session will dramatically speed up the intake process. If you do not complete the paperwork before your first session, please arrive 15 minutes early for first appointment to do so.
Attached below is the paperwork that all new clients will need to complete. Again, we recommend that you print these documents and complete them before your first session in order to simplify the intake process.
You will need to complete all documents below. The “Self” questionnaire explores how you feel the problem is affecting you. The “Other” questionnaire explores how you feel the problem is affecting another person or people you are concerned about.
In addition, your clinician may have specific information about your individual registration needs.
